Insurance Disclosure
Our goal is to help you maximize your dental insurance benefits. As a courtesy, we are happy to bill your dental plan for services. Although we file claims for you as a courtesy, your dental insurance policy is a contract between you, your employer and your insurance company. We are not a party to that contract.
Your treatment plan is individually tailored, and is not on based on your dental insurance benefits or lack of benefits.
Not all services are covered benefits in all contracts, some insurance companies arbitrarily select certain services they will not cover. By having our office process your insurance forms, it is important that you understand that this does not eliminate your financial obligation for your treatment.
It is your responsibility to thoroughly understand the coverage and exceptions of your particular insurance policy. Coverage issues or questions can only be addressed by your employer or plan administrator. We cannot act as a mediator with your insurance company. Capital City Dentistry is a United Concordia Preferred Provider, please note that some policies have different benefits for In Network and Out of Network Providers.
Insurance payments are ordinarily received within 30-60 days from the time of filing. Capital City Dentistry files all claims on the date services are rendered. If your insurance company does not pay within 120 days of the date your claim was filed or denies your claim for any reason you are responsible for payment in full at that time.
Most often, insurance companies will send their payment directly to your provider. On the rare occasion that your insurance company sends you the payment for services rendered, you will then be responsible for payment in full and must notify Capital City Dentistry immediately upon receipt of payment.
It is your responsibility to notify Capital City Dentistry if any of your insurance information has changed prior to your appointment.
